We have Level 3 recognition by the NCQA as a Patient-Centered Medical Home (PCMH) and offer our patients high-quality integrated care including primary medical care (adult medicine, pediatrics, geriatrics), dental care, psychiatry, optometry, behavioral health services and In-house Pharmacy regardless of their insurance status or ability to pay. We have an opportunity for a Pediatric APC (either ARNP or PA-C) to join our team. We are a mission-driven Community Health Center focused on providing patient-centered, quality-driven, whole-person healthcare services accessible to everyone in our community. You will have the chance to develop and cultivate patient relationships through the management of your own patient panel all while being supported by a diverse, integrated care team including physicians, nurses, dentists, and behavioral health providers.
• What we’re looking for: A pediatric APC passionate about community health and practicing as a critical member of a truly integrated primary care team A healthcare professional committed to providing care for everyone in our community Comfort/experience with Electronic Health Record use, specifically Epic Highlight your training, especially if you have experience in community health
