The Case Manager coordinates services to maximize consumer recovery and independence for individuals with mental illness. They perform psychosocial assessments, facilitate treatment plans, and provide community-based support while ensuring compliance with agency standards.
A bachelor’s degree in a human services field is required, with professional licenses like LBSW or LMSW preferred. Candidates must have a valid Michigan driver’s license and prior experience in a mental health or rehabilitation setting is preferred.
Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness. In a residential or clinical setting, provide clinical and program consultation to direct care staff. Ensure case management compliance with agency standards and the practice model. Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community. Implement strategies for obtaining services and supports that are goal-oriented and individualized.
