The Medical Social Worker provides case management services to patients, collaborating with physicians to navigate acute care episodes. They monitor resource utilization and assist patients and families in preparing for safe and timely discharges.
Candidates must hold a minimum of a Bachelor's degree in Social Work and a current license in the state of West Virginia. At least one year of clinical social work experience in a hospital or community setting is required.
Minimum of Bachelors Degree (BSW) from approved school of Social Work, holds a current license in the state of West Virginia. No temporary permits accepted.
Certifications in Care Management preferred, not required. BLS required.
MSW Social Worker, Licensed (MSW) Social Worker should have one (1) year of clinical Social Work experience, community bases and/or hospital based.
