Primary Care Nurse Practitioner - Stockton / Modesto / Amador

• Role Overview We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.
• What You’ll Do Routine & urgent assessments; chronic disease management Post-discharge follow-ups, med reconciliation, labs/diagnostics Coordinate with facility staff, specialists, families/POAs Same-day documentation; CMS/regulatory compliance Schedule & Setting Full-time position with a structured weekday schedule.
• You'll spend 3 days delivering on-site care across Stockton, Modesto, and Amador, with strategically clustered routes designed to minimize travel and maximize time with patients.
• The remaining 2 days are remote, giving you the flexibility to work from home for administrative and documentation tasks.

Qualifications
