About the role

The Step-Down Navigator implements processes to meet patient needs for a continuum of care by discussing outpatient services with eligible patients. They are also responsible for communicating eligibility to leadership and maintaining accurate data records in Midas.

Responsibilities

  • Glen Oaks Hospital is a 54-bed private mental health hospital in a relaxed setting in Greenville, Texas.

  • Our greatest asset is the staff, comprised of psychiatrists, professional nurses, social workers and therapists.
  • These individuals work as a team to uphold Glen Oaks Hospital’s reputation for excellence in helping patients resolve mental health and addiction problems.
  • For over 30 years, Glen Oaks Hospital has been a leader in the field of mental health and chemical dependency care, and our staff is dedicated to researching the latest treatment approaches and applying this to our overall treatment philosophy.
  • The Step-Down Navigator is responsible for implementing a revised process of meeting patient needs for a continuum of care.
  • They will meet with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment.
  • The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes.
  • They will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed.
  • It is expected that the Step-Down Navigator will identify implementation barriers and evaluate the new process with the facility leaders using the data.
  • A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.

Hours

Day shift, Monday through Friday, 8am to 5pm.

Qualifications

Education

Bachelor's degree in mental health or behavioral sciences field, preferred.

Experience

Two years minimum experience in psychiatric / mental health field, preferred.

Requirements

A bachelor's degree in a mental health or behavioral science field is preferred, along with at least two years of experience in the psychiatric or mental health field. Candidates must also hold CPR BLS certification and complete Handle with Care and Verbal De-escalation training.
