You’ll Do: Provide hands-on support and job coaching in Community-Based, Day Hab or Residential program settings Help individuals develop workplace, behavioral, and daily living skills Partner with Case Managers and teams to implement employment and support plans Track progress through documentation, reports, and data collection Support individuals with transportation, community activities, and job placements Assist with personal care and mobility needs as required Maintain a safe, structured, and supportive environment Serve as a positive role model and advocate for those you support

Expect: This is an active, hands-on role working in either Residential, Day Hab or Community-Based program site. You may support individuals who have behavioral challenges and/or are medically complex which you will be trained for. Flexibility, teamwork, and a positive attitude are key.
