The physician will provide comprehensive primary care to adults and children, including routine physicals, screenings, and the management of acute and chronic conditions. They are also responsible for collaborating with a multidisciplinary team, supervising mid-level providers, and maintaining accurate patient records in the EMR system.
Candidates must be graduates of an accredited medical school and have completed an approved residency program in Family Medicine. A valid, unrestricted Pennsylvania medical license, DEA certification, and board eligibility are required.
ChesPenn is seeking a dedicated and patient-centered Family Medicine Physician to provide comprehensive primary care to adults and children at our Chester, PA location. This role is ideal for a physician who thrives in a collaborative, community-focused environment and is passionate about serving medically underserved populations.
Graduate of an accredited medical school Completion of an approved residency program in Family Medicine Unrestricted Pennsylvania medical license Board eligibility required; board certification preferred DEA certification Current COVID-19 and flu vaccinations required Experience in ambulatory primary care or community health preferred
Knowledge, Skills, and Abilities Strong written and verbal communication skills Excellent diagnostic and critical thinking abilities Positive, inclusive, and patient-focused demeanor Familiarity with trauma-informed care principles preferred Ability to work effectively as part of a multidisciplinary healthcare team Valid driver’s license and access to a licensed automobile preferred Ability to meet approved physical and medical standards
Scope of Responsibilities
Provide routine and preventive care, including physical exams and screenings Manage acute and chronic conditions in an outpatient setting Conduct follow-up visits for patients referred to specialists Educate patients on health maintenance and ongoing care Collaborate with nurses, medical assistants, and administrative staff Supervise nurse practitioners and physician assistants Participate in emergency call coverage rotation, including weekends and holidays Maintain patient confidentiality and professional conduct at all times Perform additional duties as assigned by supervisor
Maintain productivity levels in accordance with BPHC grant standards Meet targets for patient encounters and visits per patient annually
Document patient care accurately in the Electronic Medical Record (EMR) system Build relationships with community agencies and hospitals Lead or support medical and public health initiatives as needed Contribute to the development of the annual healthcare plan Engage in continuing medical education and professional development
