The Case Manager II provides primary support and case management for individuals with mental illness and co-occurring disorders to help them live independently. They collaborate with a multidisciplinary team to deliver integrated services and monitor progress toward recovery goals.
Candidates must have a high school diploma with two years of experience, an associate degree with one year of experience, or a bachelor's degree in social services. A valid driver's license and the willingness to use a vehicle for client transport are required.

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Expected starting wage range is $24.59 - $29.64. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

This is a “whatever it takes” model of intensive and assertive community-based wraparound services designed to help 100 clients stabilize and avoid further episodes of homelessness, hospitalization and incarceration. Telecare’s IFSP program will consist of three multidisciplinary teams that include clinicians and professionals from psychiatry, nursing, substance abuse, peer support, housing, employment/education, and other relevant fields. Based on their areas of expertise, team members will collaborate and deliver integrated services, monitor progress toward goals, and adjust services over time to meet clients’ changing needs.
