The provider will perform physical exams, diagnose and treat acute and chronic conditions, and develop individualized care plans for residents in long-term care facilities. They will also collaborate with interdisciplinary teams and educate patients and families on treatment goals.
Candidates must hold an active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license in Texas. A minimum of 6 months of experience in long-term care or a skilled nursing facility is required.
