The Hospice Chaplain provides spiritual care and bereavement support to patients and their families while coordinating with an interdisciplinary team. They are responsible for conducting spiritual assessments, facilitating visits from community clergy, and documenting all services provided.
Candidates should possess a degree from an accredited seminary or school of theology and have experience working with death and dying. Completion of one unit of Clinical Pastoral Education (C.P.E.) is preferred, along with a valid driver's license and reliable transportation.
Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred. Experience working with death and dying, individuals/family/caregiver. 1 unit of Clinical Pastoral Education (C.P.E.) preferred. Ability to work as member of interdisciplinary team/group. Comfort in an interfaith setting. Ability to accept different lifestyles, cultures, beliefs, and values. Ability to network with community clergy and congregations. Knowledge of and commitment to hospice philosophy of care. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order. Hospice experience preferred. Essential Job Functions/Responsibilities The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Horizon Home Health and Hospice, either directly or through coordination of care with other spiritual counselors. Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner. Assures initial and on-going bereavement assessments of patients and families/caregivers in the hospice program is completed in a timely manner and an individualized bereavement plan of care is implemented. Assist social workers as needed. Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs. Provides direct spiritual care to patients and families/caregivers. Provides bereavement counseling: emotional, psychosocial, and spiritual support and services provided before and after the death of the patient to assist with issues related to grief, loss, and adjustment. Serves as a liaison and support to community chaplains and spiritual counselors. Facilitates visits by community chaplains, spiritual counselors, local clergy, and others as necessary to help support the patient’s spiritual needs. Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision. Documents direct services and ongoing communication with community chaplains and spiritual counselors. Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group. Provides consultation, education, and support to the interdisciplinary group on spiritual care.
