This role is critical in delivering community-based, recovery-oriented psychiatric care to individuals with serious mental illness. This is a highly mobile, community based position where services are delivered in a variety of settings - including members homes, community locations, and clinical environments. The ideal candidate is comfortable working in the field, collaborating in real-time with a multidisciplinary team, and meeting members where they are to support engagement and outcomes. As part of a multidisciplinary ACT team, you will provide comprehensive psychiatric assessment, diagnosis, medication management, and treatment planning, while collaborating closely with case managers, therapists, and medical providers to support whole-person care. Key Responsibilities Conduct psychiatric evaluations, reassessments, and ongoing medication management Develop and oversee individualized treatment plans Provide risk assessments, patient education, and family interventions Collaborate with ACT team members to ensure continuity of care across field and clinical environments Coordinate care with primary care, hospitals, and external providers Monitor labs, medication effectiveness, and adjust treatment as needed Ensure timely and compliant clinical documentation in the EHR Participate in quality improvement initiatives and ACT fidelity standards Support staff through clinical guidance, coaching, and mentorship Participate in on-call rotation to support 24/7 ACT services
Current and valid RN and APRN license in the State of Arizona. Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC).
