The Health Coach performs health screenings and utilizes motivational interviewing to support participants within the neighborhood Health Hub program. They collaborate with clinicians to design personalized health programs and facilitate referrals to primary and behavioral healthcare services.
Candidates must possess a High School Diploma or GED and at least two years of related work experience. A valid driver's license is required, and proficiency in Microsoft Office and virtual communication platforms is essential.

This position collaborates with clinicians and participants to design individual programs as well as make referrals to necessary primary and behavioral health care and other community supports.
This is a grant-Funded position funded until december 31, 4712.
