The Speech Therapist/Language Pathologist will partner with the Case Manager to provide skilled services addressing patients' breathing, speaking, and swallowing functions. This involves evaluating and treating speech, language, cognitive, and fluency disorders that impact communication while serving as an educator and advocate within the care team.
Candidates must possess a current unrestricted California Speech Therapist License, a valid CA Driver's License, and current BLS/CPR Certification. Experience in home health care and working with high acuity patients or interdisciplinary teams is highly preferred.
