The Advanced Practice Provider will provide comprehensive primary care services to adult patients, including diagnosing and managing acute and chronic medical conditions. They will also perform preventive care visits, order diagnostic tests, and coordinate care with specialists and ancillary services.
Candidates must be a licensed Nurse Practitioner or Physician Assistant in New York with national board certification. BLS, ACLS, and DEA certifications are required, along with a preference for 1-2 years of experience in internal or primary care medicine.
We are seeking a compassionate and motivated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join a busy, well-established internal medicine group practice. The APP will work collaboratively as part of a six-physician team, providing high-quality, patient-centered care within a supportive and collegial clinical environment. This position is ideal for an APP who values continuity of care, collaborative physician partnership, and building long-term relationships with adult patients in a supportive, team-based environment. Key Responsibilities Provide comprehensive primary care services to adult patients under physician collaboration Evaluate, diagnose, and manage acute and chronic medical conditions Manage chronic disease states including diabetes, hypertension, obesity, and cardiovascular risk factors Perform preventive care visits, annual wellness exams, and Medicare wellness visits Order and interpret diagnostic tests and labs Prescribe medications in accordance with New York State regulations Educate patients on disease management, lifestyle modification, and preventive health Document clinical encounters accurately and efficiently in the electronic medical record (EMR) Coordinate care with specialists, hospitals, and ancillary services as needed Support quality initiatives and evidence-based care standards
Licensed Nurse Practitioner in the State of New York (or eligible) National board certification required BLS/ACLS/DEA required Minimum 1–2 years of experience in internal medicine, primary care, or adult medicine preferred (new graduates with strong clinical rotations will be considered) Strong clinical assessment and diagnostic skills Commitment to patient-centered, high-quality care Ability to work collaboratively in a physician-led practice
