The Care Coordinator provides education, resources, and support to individuals managing mental illness or substance use disorders to promote recovery. They also facilitate transportation and assist clients in developing interpersonal and independent living skills.
Candidates must hold a bachelor's or master's degree in a human services field, or possess specific professional certifications or relevant experience. A strong background in delivering services to individuals with mental health or developmental disabilities is required.
The services provided by a Care Coordinator (CC) focus on helping individuals and family develop skills, access resources and manage illness to be successful in the living, working, learning and social environments of their choice. CC’s teach, model and practice skills with person(s)-served to promote recovery and wellness.
