The Hospice RN Case Manager is responsible for coordinating and delivering high-quality hospice care, focusing on symptom management and care planning. They will manage a caseload of patients and collaborate with an interdisciplinary team to ensure comfort and quality of life.
Candidates must have a current California RN license and preferably at least one year of hospice experience. Strong clinical assessment skills and the ability to work independently are also required.
30 AM – 5:00 PM | Field Position Hospice Case Managers are vital members of our care team, providing compassionate, skilled nursing support to patients and families during end-of-life care. As a Hospice RN Case Manager, you will oversee patient care, coordinate interdisciplinary support, and ensure comfort, dignity, and quality of life for every patient you serve.

The Role: Total Case Management
In this position, you are not just making visits—you are managing the entire clinical ecosystem of your patients. You are the primary point of contact for the family and the central hub for the Interdisciplinary Group (IDG).
Preferred* Employee Living Location: Area Central to Los Angeles and Northern Orange County (such as Montebello and surrounding cities)
