Candidates must hold at least a Bachelor's degree in a related field and possess at least one year of experience working with the identified population. A current QMHP-T, QMHP-A, QMHP-C, or relevant clinical licensure is required.

The Mental Health Professional plays a vital role in our mission to deliver coordinated, individualized care to our communities. As a Mental Health Professional, you will: Case Management & Documentation: Maintain accurate and timely case records per licensing and Medicaid standards, ensuring continuity and quality of care. Crisis Intervention: Provide direct support during crisis situations, helping clients to navigate and stabilize through effective case management. Service Planning & Coordination: Coordinate and develop Individualized Service Plans with input from team members and stakeholders to align treatment goals. Trauma-Informed Counseling: Deliver person-centered, trauma-informed, and psycho-educational interventions designed to support treatment objectives. Individual & Family Therapy: Facilitate individual, family, and group counseling sessions, utilizing approved credentialing and skill set. Collaborative Communication: Maintain regular contact with case professionals, such as school personnel, social services, and juvenile justice representatives, ensuring cohesive client care. Professional Case Presentations: Provide case updates and presentations in formal settings such as FAPT meetings, IEP sessions, and court appearances.
