What You’ll Do

Completes an initial, comprehensive and ongoing assessments of patient and family to determine home health or hospice needs.

Provide respectful, compassionate care in patients’ homes.

Develop patient focused care plans that establish goals based on nursing diagnoses, and incorporate rehabilitative nursing actions.

Collaborate with interdisciplinary team members.

What You Bring

  • Active nursing certification (state-licensed).
  • Valid driver’s license, reliable transportation, and current auto insurance.
  • Strong communication skills and a team-oriented mindset
  • Ability to work independently in home settings
