About the Role

• The MDS/RN at Country View, plays a critical role in ensuring the delivery of high-quality patient care through comprehensive assessment and care planning.
• This position is responsible for conducting thorough Minimum Data Set (MDS) assessments to evaluate residents' health status, functional capabilities, and care needs.
• The MDS/RN collaborates closely with interdisciplinary teams to develop individualized care plans that promote optimal health outcomes and regulatory compliance.
• This role requires meticulous documentation and adherence to federal and state guidelines to support reimbursement processes and quality improvement initiatives.
• Ultimately, the MDS/RN serves as a key liaison between clinical staff, residents, and families to foster a supportive and effective care environment.

Minimum Qualifications

• Current and valid Registered Nurse (RN) or LPN license in the state of Missouri.
• Certification in Minimum Data Set (MDS) assessment or willingness to obtain within a specified timeframe.
• Strong knowledge of federal and state regulations related to long-term care and MDS requirements.
• Proficiency in electronic health records (EHR) and clinical documentation systems.

Preferred Qualifications

• Certified MDS Coordinator credential (e.g., Certified MDS Specialist).
• Experience with quality improvement programs and regulatory survey processes.
• Advanced training in geriatric nursing or long-term care management.
• Familiarity with reimbursement methodologies such as Medicare and Medicaid.
• Excellent communication and leadership skills to effectively coordinate interdisciplinary teams.

Responsibilities

• Perform comprehensive MDS assessments in accordance with federal and state regulations to accurately capture resident health and functional status.
• Develop, update, and coordinate individualized care plans based on assessment findings and interdisciplinary team input.
• Collaborate with nursing staff, therapists, social workers, and physicians to ensure continuity and quality of care.
