About the Role

• We are seeking a highly skilled and dedicated MDS LPN or RN to join our team at Country View Nursing.

Minimum Qualifications

• Active LPN or RN license in the state of Missouri
• Minimum of 2 years of experience in a long-term care setting (preferred)
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Excellent organizational and time management skills

Preferred Qualifications

• Experience with MDS assessments and care planning
• Experience with electronic medical records systems

Responsibilities

• Conducting patient assessments and developing individualized care plans
• Administering medications and treatments as prescribed by physicians
• Monitoring and documenting patient progress and communicating with healthcare team members
• Providing education and support to patients and their families
• Maintaining accurate and up-to-date patient records

Skills

As an MDS LPN, you will utilize your strong communication and interpersonal skills to build relationships with patients and their families. Your attention to detail and ability to work collaboratively with healthcare team members will be essential to developing and implementing effective care plans.
