The Case Manager will help clients identify the causes of their homelessness and coordinate with service teams to develop independent living plans. They will also provide referrals for employment, medical, mental health, and other essential services to help clients achieve self-sufficiency.
Candidates must possess a Bachelor's degree in social work or a related field. Strong communication skills and the ability to manage multiple priorities effectively are required.
