About the role

The LPN will provide essential nursing care, including monitoring vital signs, administering medications, and performing wound care under the supervision of RNs and physicians. They are also responsible for documenting patient information accurately and collaborating with the healthcare team to implement care plans.

Responsibilities

Provide direct nursing care to residents in accordance with physician orders, individualized care plans, and nursing standards. Administer medications, treatments, and therapies safely and accurately. Assess residents, monitor for changes in condition, and promptly notify the appropriate healthcare provider. Accurately document resident care, medication administration, and clinical observations in the electronic health record. Supervise and delegate nursing tasks to Certified Nursing Assistants (CNAs) while providing guidance and support. Assist with admissions, discharges, transfers, and resident assessments. Communicate effectively with residents, families, physicians, and the interdisciplinary care team. Perform nursing procedures within the scope of practice, including wound care, catheter care, and other treatments as assigned. Ensure physician orders are implemented accurately and completed in a timely manner. Maintain infection prevention and control practices while promoting a safe environment for residents and staff. Participate in quality improvement initiatives, staff education, and survey readiness activities. Comply with all federal, state, local, and facility policies, including resident rights and HIPAA regulations. Promote a culture of compassion, professionalism, teamwork, and resident-centered care. Perform other duties as assigned to support the nursing department and ensure quality resident care. Valid and current Licensed Practical Nurse (LPN) license. Completion of an accredited practical nursing program. Prior experience in a clinical or healthcare setting is preferred. Strong knowledge of nursing care practices, medication administration, and patient safety protocols. Good communication and interpersonal skills to interact effectively with patients, families, and healthcare staff. Ability to work under supervision and as part of a multidisciplinary team. Commitment to compassionate, patient-centered care. Flexibility to work various shifts including weekends and holidays. Requirements
