The Care Coordinator provides therapeutic case management and rehabilitation services to support the wellness and recovery of chronically homeless individuals. Responsibilities include conducting assessments, developing service plans, performing outreach, and maintaining accurate documentation in compliance with regulatory policies.
Candidates must have a high school diploma and at least 4 years of paid work experience in mental health or social services. A valid California driver's license, personal vehicle, and proof of insurance are required for extensive local travel.
The Care Coordinator, Enhanced Case Management services is responsible for developing and providing high quality therapeutic interventions and case management services in support of an individual’s wellness and recovery. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support. This position is based at our Camellia Village 42 unit independent SIL housing program in Sacramento.
This is a community-based position providing direct, face-to-face services to individuals who are chronically homeless and travel throughout Sacramento County is required.
• Sunday to Thursday and Tuesday to Saturday.
