• Description: Hospice Homecare LPN - Per Diem Saratoga&Washington Counties WEEKDAYS 8am to 4pm The Community Hospice has a wonderful opportunity for a Per Diem LPN to work as a member of our Hospice Home Care Team caring for patients and families in both the homecare and nursing home settings. Primary region to cover is Saratoga and Washington Counties.

• What you will do: Coordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and families This is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers). Training provided in end of life care.
• What you will need: Valid NYS LPN License Minimum 1 year LPN experience required Prior hospice/ home care experience preferred, but not required Strong computer skills Organization and communication skills and ability to work as part of a team Community Hospice provides a thorough orientation, preceptors and a supportive work environment. • Range for non-per diem status: $25.00-$31.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
