The Social Services Assistant supports the Social Service Director in planning and implementing programs to meet the emotional and social needs of residents. Responsibilities include coordinating care, assisting with discharge planning, maintaining progress notes, and facilitating communication between residents, families, and medical staff.
Candidates must possess at least a high school diploma or equivalent, with an Associate or Bachelor's degree preferred. Prior healthcare experience is preferred, along with strong communication, analytical, and organizational skills.
