Licensed Master Social Worker (LMSW)

This role provides biopsychosocial assessments, care coordination, discharge planning, and protective interventions for vulnerable adults. The LMSW will support APS-related cases, risk assessments, legal matters (guardianship/POA), and connect patients with essential community and medical resources. Key Responsibilities: Perform biopsychosocial assessments and initial evaluations Address safety risks, abuse/neglect concerns, guardianship, and POA Coordinate discharge planning, DME, rehab, and long-term care referrals Collaborate with care teams in ED, ICU, Med-Surg, L&D, and NICU Document assessments, plans, and interventions per standards Provide education on Advance Directives, ADA, and community resources Navigate Medicaid, Medicare, transportation, and medication assistance programs

Requirements

Master’s degree in Social Work (MSW) Washington State LMSW license (or eligibility) Experience in acute care, APS, crisis intervention, or case management preferred
