The Medication History Technician is responsible for accurately obtaining, documenting, and verifying patient medication histories upon admission, transfer, or discharge. They work under the supervision of a pharmacist to ensure medication accuracy and collaborate with healthcare professionals to reduce adverse drug events.
Candidates must have a high school diploma or equivalent and be registered as a Pharmacy Technician by the Mississippi State Board of Pharmacy. Completion of ASHP Medication History-Taking Certification and de-escalation training is required within 90 days of hire.

Job Description: Summary
Position Overview: The Medication History Technician (MHT) is responsible for accurately obtaining, documenting, and verifying a patient’s medication history upon admission, transfer, or discharge. The MHT works under the supervision of a licensed pharmacist and in collaboration with physicians, nurses, and other healthcare professionals to ensure medication accuracy and reduce adverse drug events. Expectation is for all performed duties to be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees. High school graduate or equivalent required. Graduate of a pharmacy technician training program preferred.
Must be registered as a Pharmacy Technician by the Mississippi State Board of Pharmacy.
Must possess a valid Pharmacy Technician Registration from the Mississippi Board of Pharmacy. Must possess or complete ASHP Medication History-Taking Certification within 90 days of hire/assignment. Must have de-escalation training completed by the end of position orientation (90 days); must have appropriate level of de-escalation training.
