About the role

The Nurse Practitioner will deliver comprehensive medical care in a primary care outpatient setting, focusing on health promotion and disease prevention. They will also collaborate with interdisciplinary teams and patients to optimize clinical outcomes and maintain a safe, efficient environment.

**Job Description: **

The Nurse Practitioner (APRN) will be responsible for executing medical procedures in alignment with APRN licensure standards in a primary care outpatient environment.

Key responsibilities include

  • Delivering comprehensive nursing and medical care across individual, family, and group settings, with an emphasis on health promotion, disease prevention, and holistic wellness.
  • Providing leadership and consultation within interdisciplinary care teams.
  • Effectively collaborating with patients, families, healthcare professionals, including nursing staff and physicians, to optimize clinical outcomes.
  • Identifying and addressing the unique educational and instructional needs of patients and their families.
  • Facilitating patient education to enhance informed healthcare decision-making processes.
  • Maintaining a clinically safe and efficient environment conducive to patient and staff well-being.
  • Supporting daily operational practices that are cost-effective and sustainable.
  • Engaging in continuous professional development and education to stay abreast of advancements in nursing practice.
  • Assessing personal learning needs and establishing targeted goals for professional advancement.
  • Collaborating within a Medical Group encompassing various specialties, such as family practice, pediatrics, OB/GYN, cardiology, orthopedics, post-acute care, urology, and general surgery.
  • Working in conjunction with physicians, licensed providers, LPNs/medical assistants, and administrative personnel.

Please note, candidates residing beyond a one-hour commute will not be considered.

**Qualifications

  • *
  • Master’s Degree from an accredited Adult Nurse Practitioner program.
  • Current Vermont APRN License (or eligibility to obtain one).
  • Certification as an Adult, Family, or Pediatric Nurse Practitioner from a national organization recognized by the Vermont Board of Nursing.
  • Healthcare Provider CPR Certification.
  • DEA License.
  • Eligibility for credentialing.
  • Minimum of two years of nursing experience prior to attaining an advanced degree.
  • An unblemished malpractice history.
  • Inquiry into visa support requirements for the provider.
  • Recent NPDB Self-Query Report (issued within the last 30 days).
