The Case Manager will conduct outreach to homeless individuals in Staten Island, providing assessments and connecting them to essential services like housing, detox, and medical care. They are also responsible for maintaining accurate program statistics and responding to community requests for outreach assistance.
Connect with homeless people living on Staten Island completing assessments and appropriate referrals including psych evaluations, TB and HIV testing, detox, etc. Assists clients in filling out forms for entitlements, etc. Complete psychosocial and social history when applicable. Counsel clients leading to interim, transitional and permanent housing or shelter placements Complete all paperwork as well as accurate timely statistics as required by the program Respond in a timely way to calls from the community regarding homeless individuals in all areas of Staten Island Drive and escort clients to the necessary services they require (ESU, detox, etc) using the Outreach van Participate in weekly supervision, case conferences and staff meetings QualificationsQualifications Needed: HSD/GED required with 1- 2 years experience with the homeless population, Bachelor's Degree preferred Experience with the homeless, substance abuse, and mentally ill populations is needed Excellent client engagement skills are a must Must possess a clean and valid driver's license and the ability to drive a passenger van Bilingual Spanish/English preferred Must have excellent communication skills both orally and in writing with individuals and groups in regard to complex and sensitive issues The physical ability to perform essential job functions is needed In addition, the Outreach Case Manager shall perform any duties as required to meet the needs of the program and the clients we serve.
Candidates must have a High School Diploma or GED with 1-2 years of experience working with homeless populations, though a Bachelor's degree is preferred. A clean, valid driver's license is required to operate the outreach van, along with strong communication and engagement skills.
