Provide support coordination and case management services to adults with developmental or intellectual disabilities. Assist individuals in accessing necessary medical, psychiatric, educational, and residential support services.
PCS is seeking an ID/DD Support Coordinator to provide support coordination & case management services to adults with developmental/intellectual disabilities and assure that these individuals receive appropriate services, assisting them in accessing needed medical, psychiatric, educational, residential and other support services.
A Bachelor's degree in Behavioral Sciences or related Human Services field and minimum of one-year experience working with the population is required.
Basic computer skills are needed and experience with assessment and service plan development is preferred.
Must have a valid driver's license, safe driving record, and reliable ability to transport self and others to worksites, trainings, and meetings.
Salary is determined by education & experience as it relates to the position.
