The Case Manager provides direct crisis intervention, support, and education to SMI adults and their families. They are responsible for completing needs assessments, coordinating referrals, and ensuring continuity of care across community services.
Candidates must hold a bachelor's degree in a behavioral health field and have experience working with severely mentally ill adults. A valid driver's license, reliable transportation, and a fingerprint clearance card are mandatory.

Sign-on bonus up to $2000 (limited time incentive)
We are looking for individuals to join our team as Case Manager to work with our SMI Adult population at our San Tan location. This position will provide direct service crisis intervention, support, transportation and education to families and children, independently of, or in conjunction with, other in-home client services. It completes family needs assessments, makes appropriate referrals to collaborating partners and other community services as client needs indicate, ensures the continuity of care in the provision of comprehensive services to clients and assists in coordinating communications with other departments of the agency or with collaborators in accomplishing this function. This position is Full Time and will require flexibility with scheduling client and family needs.
