The Family Nurse Practitioner provides primary care services, manages acute and chronic health conditions, and conducts health screenings for diverse populations. They also collaborate with clinical staff, provide patient education, and participate in public health initiatives.
Candidates must be licensed as a Registered Nurse and an Advanced Practice Nurse in the State of Pennsylvania. A minimum of three years of nursing experience, including at least two years as an Advanced Practice Nurse, is preferred.
PHMC serves as both a direct service provider to individuals, families, and communities across the region and an intermediary agent, managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives that require operational sophistication, strategic leadership, and deep mission alignment.

Family Nurse Practitioner will provide primary care services for adults, according to certification, in federal-qualified health center settings to diverse and underserved populations. Employee will report to the Medical Director.

• Three (3) years of nursing experience, at least two (2) of which shall have been as an Advanced Practice Nurse in a related specialty, preferred, but not required.
