The Behavioral Health Provider delivers brief individual therapy and crisis intervention within a multidisciplinary primary care team. They are responsible for conducting assessments, developing treatment plans, and collaborating with medical providers to ensure whole-person care.
Candidates must hold a current, valid California registration or license as an ASW, AMFT, LCSW, or LMFT. Proficiency in English and Spanish is required, along with strong clinical skills and familiarity with the DSM-V.
Santa Cruz Community Health (SCCH) is a multi-site, Federally Qualified Health Center (FQHC) serving Santa Cruz County residents.
