Perform initial and ongoing assessments to evaluate functional status, service needs, and risks for individuals. Coordinate care plans and communicate findings with members, caregivers, and relevant healthcare agencies.
A bachelor's degree in a health or human services field is preferred along with experience in assessment or care coordination. Candidates must possess effective organizational skills and have reliable transportation for field visits.

Effective organizational, interpersonal, communication and leadership skills. Effective computer skills with familiarity with suite of Microsoft Office programs. Reliable transportation to travel within assigned region as business requires. Valid driver’s license and car insurance as required by State law.
