The Behavioral Health Care Manager serves as the primary point of contact for members, conducting comprehensive assessments and developing person-centered care plans. They coordinate care across providers and community resources while performing in-person home and community visits.
Candidates must hold a Master’s degree in Social Work or Psychology and possess an active clinical license such as LCSW, LMFT, or LPCC. At least one year of experience in care management or behavioral health is required, along with a valid driver's license.
Behavioral Health Care Manager (ECM) – Licensed (Hybrid) Heritage Health Network (HHN) Join Heritage Health Network as a licensed Behavioral Health Care Manager (BHCM) and support high-need Medi-Cal members through our Enhanced Care Management (ECM) program. This hybrid role includes 2–3 days in the field and 2–3 days in the office, allowing for flexibility while maintaining meaningful, in-person member engagement. What You’ll Do Serve as the primary point of contact for members with behavioral health and psychosocial needs Conduct comprehensive assessments and develop person-centered care plans Coordinate care across behavioral health providers, medical teams, and community resources Conduct in-person home and community visits Support transitions of care and post-discharge follow-up Provide education, coaching, and trauma-informed support Maintain accurate documentation and meet program requirements
Education: Master’s degree in Social Work or Psychology required Licensure: Active Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or Licensed Professional Clinical Counselor required Experience: 1+ years in care management, behavioral health, or related setting (Medi-Cal/ECM preferred) Bilingual English/Spanish preferred Strong communication and care coordination skills Knowledge of behavioral health, SDOH, and community resources Comfortable with field work and home visits Valid driver’s license, insurance, and reliable vehicle
