Community Health Worker (Part-Time 20 hours per week)

The position will also be responsible for

• Provide individuals with resources to obtain health insurance coverage and health care services. Help patients with educational resources that promotes healthy living. Incumbent may travel via personal vehicle throughout the community and between other Clinical facilities and departments. Ability to interact in community settings, such as health fairs, with up to 40% travel. Drive the Mobile Health Van as needed Medical Assistant as needed. Assist the medical provider in providing high quality, efficient and service-oriented patient care demonstrating our core values. Under the direction of a Physician, Nurse Practitioner or lead, assist with direct patient care within the scope of a Medical Assistant. Measure and record vital signs, turn over exam rooms and answer phone calls, as needed. Assist patients with scheduling appointments, perform data entry such as updating medical histories and contact information as needed. Uphold all HIPAA (Health Insurance Portability and Accountability Act) and other regulatory compliance and safety requirements.

• Experience: Minimum of 6 months of experience as a Medical Assistant. Prior experience utilizing Epic electronic health record system Community Clinic experience.
• Knowledge/Skills/Abilities:
• Required: Strong organization and follow-up skills. Ability to multi-task and prioritize. Knowledge of computer software programs and basic office equipment. Demonstrates the ability to read, write and speak English and Spanish without an interpreter. Demonstrates interpersonal/cultural sensitivity and work with a diverse and often underserved population. Effective written and verbal communication across all levels of the organization and with vendors. Highly organized with strong time management skills, being able to perform simultaneous tasks, prioritize them effectively and follow through quickly. Proficiency with EMR systems. (EPIC experience preferred.) Communication skills consistent with the facilitation of health and wellness topics to individuals, small and large groups of community residents. Reliable vehicular transportation to travel to community events.
