The Social Service Coordinator is responsible for assessing patient needs and coordinating access to medical and psychosocial programs. They also collaborate with interdisciplinary care teams to evaluate patient progress and maintain community partnerships.
Candidates must hold a Bachelor's degree in a Health and Human Services-related field or possess a current Nevada State Social Work Licensure (LSW). At least one year of experience in an outpatient or inpatient behavioral health setting is preferred.
The Social Service Coordinator (SSC) shall have primary responsibility for connecting patients to social resources and coordinating access to medical, psychosocial programs and social case management.
