The Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of the patient’s plan of care from admission through post-discharge. This role involves utilizing clinical knowledge to coordinate and implement discharge plans that ensure seamless transitions of care and achieve quality, cost-effective patient outcomes.
Candidates must be a Registered Nurse with a Bachelor of Science in Nursing (BSN) and hold current licensure in the District of Columbia. A minimum of 2-3 years of recent acute care experience is required, and Case Management Certification is preferred.
