Perform comprehensive medical, psychiatric, and socioeconomic assessments to develop and implement individualized care plans for high-risk patients. Coordinate multidisciplinary care and conduct outreach visits in non-traditional settings such as homes and streets.
Requires a graduate degree from an accredited Nurse Practitioner or Physician Assistant program with active Texas licensure and at least 2 years of independent experience. Must hold current BLS certification and possess strong clinical assessment and communication skills.

and Essential Job Functions: Perform comprehensive assessment including, medical, psychiatric, and socioeconomic needs and work collaboratively with the patient to develop and implement a comprehensive care plan. Conduct follow up visits when and where needed, as well as address urgent and acute complaints. Revise care plan as needed, and collaborate with patient and community providers to develop and implement a transition to community based primary care when appropriate Collaborates with care team in overseeing and coordinating the multidisciplinary care delivered to all patients, to optimize quality patient care. Reviews patient medical charts with team physician with diagnosis, treatment and care management. Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Health. Provides care education to patients and families. Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores. Maintains a collaborative environment with providers, clinical staff, and support staff. Supports academic, educational and research endeavors. Performs other job related duties as assigned.
Applicant Requirements Graduate of an accredited Nurse Practitioner program with an active Texas License Must have current Texas APRN license with ACNP certification OR Graduate of an accredited Physician Assistant Program with an active Texas License Valid, unrestricted DEA Basic Life Support (BLS) certification Minimum of 2 years working independently as an advanced practice provider Must be proficient in documentation and billing in an electronic medical record system Must be able to thrive in a fast paced environment Must be able to collaborate with other members of the healthcare team Must be able to demonstrate a working knowledge in area assigned, and competency in assessment techniques, diagnosis and treatment processes that apply to the assigned medical specialty area Must have good interpersonal, verbal and oral communication skills to work effectively with other health care professionals Must be self-directed and take initiative to develop and participate in community activities and medical outreach programs pertaining to the medical specialty area Must be able to demonstrate patient centered/patient valued behavior Must be data driven and quality metric focused Must be comfortable working in a large public/county facility
