The Adult Case Manager maintains a caseload of consumers, completing comprehensive assessments and developing individualized treatment and crisis plans. They coordinate services, advocate for client entitlements, and spend significant time in the community providing direct support.
Candidates must possess a bachelor's degree in a human services field with at least one year of experience serving individuals with mental illnesses. Alternatively, a high school diploma or GED with four years of relevant experience or education is accepted.
• Bachelor’s degree in human services field, plus as least one year of full-time experience serving people with mental illnesses (serving SDMI adults is preferred) OR individuals with other educational backgrounds who, as providers, consumers, or advocates of mental health services have developed the necessary knowledge and skills (graduation from high school or GED and a combination of relevant experience and/or education equaling at least four years.)
Wage = $21.84+ DOE
