The Supports Coordinator develops and maintains individual support plans using a person-centered approach while assisting individuals in selecting qualified service providers. Qualifications Position Qualifications: Required: Preferred: Education Bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social services. OR Two years’ experience as a County Social Service Aide 3 and two years of college level coursework, which includes at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services. OR Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services AND one-year experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions. Experience One-year experience working with individuals with disabilities. Licenses, Certificates or Clearances Act 33/34 and FBI Clearances. Valid Driver’s License. Car Insurance Coverage Verification. Special Skills, Knowledge and Abilities Computer competency with ability to learn agency software. Knowledge of Community Resources.
