The Role

This role offers a rewarding opportunity to provide compassionate wound care nursing in a home health setting. Key Responsibilities

Qualifications

  • Associate degree or higher
  • 1+ years of experience in ANY of the following:
  • Home Health Care
  • Home Health
  • 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • Practice Partner EMR
  • Valid Licensed Practical Nurse (LPN) certification/license in the state of Illinois (IL)
  • Valid Certified Wound Ostomy and Continence Nurse (CWOCN) certification/license
  • Valid Regular Driver License (DL) certification/license in the state of Illinois (IL)

Requirements

Candidates must hold an Associate degree or higher and possess a valid Illinois LPN license, CWOCN certification, and a driver's license. At least one year of experience in home health care and proficiency with EMR systems are required.
