Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health SLP (Speech Language Pathologist) to work with our Home Health team to deliver individualized care to patients in their homes in Naples, Immokalee, Marco Island, and surrounding areas.

Schedule: PRN role; 5-10+ home health visits per week
What you'll Do
Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to identify the client’s level of functioning, swallowing, and communication ability.
Develop a plan of care in consultation with the physician and other care team members.
Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
Instruct patients and families on using special educational strategies and techniques to improve sensory-motor development, perception, memory, language, and cognition.
Prepares clinical and progress notes of services provided, patient activities and condition and submit in a timely manner.
Collaborate with the interdisciplinary team to ensure seamless, holistic care

Must hold an active Florida SLP license and have graduated from an accredited speech pathology or audiology program. Previous home health care experience is preferred, along with proficiency in EMR systems like Homecare Homebase.
