Perform initial and ongoing patient assessments, including OASIS documentation, to develop and implement individualized speech therapy plans. Collaborate with the interdisciplinary team to provide patient and family education on swallowing, communication, and cognitive strategies.
Must be a graduate of an accredited speech pathology or audiology program and hold an active Florida SLP license. Home health experience and familiarity with Homecare Homebase EMR are preferred.
