The Community Based Skill Builder provides in-home supportive therapy to older adults using the PEARLS model to address depression and anxiety. They are responsible for coordinating care, maintaining accurate clinical documentation, and facilitating resource referrals.
Candidates must hold a Bachelor’s degree in Social Work or a related human services field. Experience working with older adults and strong organizational skills are required for this role.
The UPLIFT Program provides in-home supportive therapy to adults age 55 and older who are experiencing symptoms of depression and anxiety. The Community Based Skill Builder works directly with clients in their homes, delivering services using the evidence-based PEARLS model of care. This role is ideal for someone passionate about supporting older adults, community-based work, and collaborative care.
