The physician will provide comprehensive primary care, including history and physical examinations, for underserved populations in an integrated health center. Responsibilities include managing acute and chronic conditions, reviewing diagnostic results, and mentoring the care team.
Candidates must be a graduate of an accredited medical school with a NYSED MD/DO license and board certification in Family or Internal Medicine. Required certifications include DEA registration, BCLS, ACLS, PALS, and an active NPI.
Mentoring and supporting the care team to enhance professional growth and team efficiency. Perform history and physical examinations in accordance with current standards of professional practice, regulatory requirements and organizational policies. Delivering patient-centered primary care while integrating preventive, chronic, and acute care strategies. Reviewing and following up on labs, imaging, referrals, and hospital/ED transitions. Documenting clinical encounters accurately in the electronic health record. Assess patients’ condition and age specific medical needs; sets goals; prescribes appropriate clinical interventions to meet those goals that reflect the physical, psychological, social, cultural and rehabilitative needs of the patient. Provide complete and appropriate documentation regarding clinical observations, diagnoses, treatment objectives, and patient’s response to treatment regimens. Will utilize an outcome-oriented approach. Prescribe appropriate laboratory/diagnostic procedures related to the patient’s health status and facilitates appropriate treatment interventions predicated on results of these procedures. Performs direct patient care and procedures in accordance with SDH policies and procedures.
A graduate of an accredited medical school. Currently registered to practice as a Physician in New York State with a specialty in Family Medicine or Internal Medicine. Recent experience in an out-patient care setting and experience in managing care of the substance use population.
NYSED MD/DO license. Board Certified Valid DEA registration. BCLS/ACLS/PALS certifications. eMEDNY Provider or willingness to enroll as a “Provider” upon hire. Active NPI.
This is a full-time exempt role with a compensation of $210,000 - $265,000 annually with a full benefits package. Benefits include medical, dental, and vision insurance, 25 days of paid time off, access to a retirement account with an employer to match, and tuition reimbursement. The compensation offered for this role is dependent on experience and credentials. This clinic is located inside the Richard Pruss building in the Port Morris section of the South Bronx and has a regular schedule of Monday-Friday 9am-5pm.
