This role involves working in community settings to assess client needs, plan and link appropriate services, and advocate for comprehensive support based on identified needs. The coach coordinates and provides services within an Individualized Recovery Plan (IRP) while interacting with individuals diagnosed with severe and chronic mental illness or substance abuse.
Candidates must possess either a High School diploma or equivalent with four years of relevant experience, or an Associate’s degree in a mental health or human services discipline with two years of relevant experience. A valid Connecticut driver’s license is required, and the employee must obtain a Public Service License within 90 days.
