The Nurse Practitioner will conduct in-home visits to perform patient assessments and manage chronic conditions. They are also responsible for coordinating care with physicians and caregivers while maintaining accurate documentation in the EMR.
Candidates must be a licensed Nurse Practitioner in Pennsylvania. Experience in primary care, geriatrics, or home care is preferred along with strong clinical and communication skills.

Fee for service $60 - $80 per encounter. Mileage is reimbursed.
