About the role

The APRN/PA will conduct comprehensive medical evaluations, formulate treatment plans, and provide ongoing care for patients within an infectious disease setting. Responsibilities also include interpreting laboratory tests, prescribing medications, and maintaining accurate clinical documentation in compliance with regulatory standards.

About the Role

We are seeking a physician who provides reasonable and prudent medical care, exercises sound clinical judgment grounded in best‑practice standards and evidence‑based medicine, and delivers care that aligns with their professional licensing, training, and credentialing.

Key Responsibilities

Conducts comprehensive medical evaluations, education, and treatment.

Maintains patient confidentiality in compliance with agency, state, and HIPAA laws.

Assesses medical and psycho-social histories, performs physical exams, identifies problems, formulates treatment plans, provides education and referrals, and evaluates intervention effectiveness.

Orders and interprets laboratory tests, reports communicable diseases to the Health Department as per protocol.

Prescribes appropriate medication.

Implements standardized evaluations or assessments with a multidisciplinary team.

Ensures complete, timely, and accurate documentation.

Understands medical emergency procedures and the location of emergency supplies.

Participates in continuing medical education to enhance professionalism and quality of care.

Provides on-call coverage as scheduled.

Maintains good working relationships with community agencies.

Attends and participates in team conferences, huddles, and required trainings.

Required Qualifications

Education

Degree (Nurse Practitioner or Physician Associate) from an accredited Nursing or Physician Associate school. CPR Certification required.

Experience

Board Certification or Board Eligibility. (Board certification must be attained within the timeframe dictated by one’s specialty in order to remain on staff at the CS-HHC if one was originally Board Eligible at the time of one’s hire.) Connecticut Licensure. Must have at least 5 years of clinical experience in outpatient internal medicine or family practice. Prior experience working in addiction medicine preferred.
