About the role

The LPN is responsible for providing direct patient care, administering medications, and monitoring patient symptoms in an acute care setting. They must collaborate with RNs and the interdisciplinary team to implement care plans and maintain accurate clinical records.

We are currently seeking a dedicated LPN to join our 4 Main Acute Care Department.

This is a Casual position that requires a minimum of 4 shifts per 6-week schedule.

Minimum of 2 weekend shifts per 6-week schedule.

Prior LPN hospital experience is preferred.

Responsibilities

Assume responsibility for patient assignments. Contribute to the universal unit activities including, but not limited to, responding to call lights and telephones, passing meal trays and transporting patients. Constructively receive feedback and direction. Observe and record patient's symptoms, condition, and/or reactions to medications and treatments and report results to the RN in charge. Provide appropriate, direct patient care. Give injections, take vital signs, perform basic diagnostic tests, observe patients, dress wounds and administer medication. Establish appropriate and effective communication strategies. Participate in health care conferences. Take action to improve knowledge, skills and performance based on feedback or on self-identified developmental needs. Suggest and support changes within the department. Communicate effectively with patients and families about hospital stay and home care concerns. Instruct and demonstrate care. Discuss any patient or family concerns with RN in charge so that issue can be appropriately addressed. Request assistance and reviews relevant policies and procedures prior to performing new or challenging skills. Maintain current CPR certification at Health Care Provider level. Participate in self-review as requested by direct supervisor. Participate in project and committee work. Assist the RN and interdisciplinary team in evaluating, planning, and implementing plans of care. Serve as a preceptor for new staff members. Establish professional and respectful interpersonal relationships. Identify and communicate learning needs to the unit director/clinician/supervisor. Perform assigned work in a timely and productive manner. Comply with established medication use policies. Demonstrate awareness of behavior on the efficient functioning of the department. Assure equipment malfunctions are reported to the appropriate department. Maintains accurate patient clinical records, executes physicians orders, and maintains accurate problem oriented nurses notes which reflect conditions, response to treatment, progress and re-assessment in accordance with accepted nursing standards. Strictly adhere to all policies and practices relative to patient care and hospital-related information. Serve as a resource for staff and other disciplines. Request assistance to help in planning and prioritizing activities as needed. Comply with safety policies and procedures including standard precautions.

Completion of a Practical Nursing Program approved by the State Board of Nursing. Excellent communication skills necessary to effectively interact with patients, family members, visitors and staff members. Basic computer skills necessary to input patient care orders and medical record documentation. 2 Years recent/relevant experience required.

Licensure, Certifications, and Clearances

CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire. Act 33 Child Clearance w Renewal. Act 34 Criminal Clearance w Renewal. Act 73 FBI Clearance. Basic Life Support OR Cardio Pulmonary Resuscitation. Licensed Practical Nurse license.

Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) Licensed Practical Nurse (LPN) Act 34 *Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.

Candidates must have a Practical Nursing Program completion and a valid LPN license. A minimum of 2 years of recent relevant experience and current CPR certification are required.
